Ridgeview Transitional Rehabilitation provides short-term rehabilitation and skilled nursing for people recovering from an injury, planned surgery or illness. A Care Transition Manager coordinates services during stays intended to help guests return home. Rehabilitation therapy and nursing are available daily, and therapists are available seven days a week. The physician determines therapy type, intensity and frequency with input from the guest, family and professional team members.

Can therapy help prepare for returning home?

Occupational therapy may include an evaluation of the guest’s home, recommendations for adaptive equipment and training in its use, as well as guidance and education for family members and caregivers.

Individual care plans start with each guest’s goals and current condition, then identify interventions. Ridgeview uses an interdisciplinary process that draws on professional expertise and knowledge from the guest and family.

Skilled nursing is available around the clock when recovery or rehabilitation requires it. Nursing follows the physician’s orders, and the interdisciplinary team supports the transition into care and then back home or to another facility, as appropriate.
