The LifeWorks Rehab® Recovery Map and Personal Report Card are tools for patients and families to track daily progress. The Recovery Map sets out the treatment plan from admission through discharge.

Long-term care is provided by a team that includes physicians, registered nurses, certified nursing assistants, therapists and dietitians.

The tracheostomy unit has tracheostomy-trained nurses on staff around the clock, respiratory therapists onsite and a pulmonologist on call. Speech, occupational and physical rehabilitation therapy may also be integrated into care when needed.

LifeWorks Rehab® begins with evaluations of medical and functional status. The recovery team then reviews needs and goals with the patient and develops an individualized recovery plan.

Rooms & living spaces

Patients may stay in a private or shared room. Each room has an ensuite bathroom with an accessible shower or tub, and staff are available around the clock to assist with needs and everyday tasks such as meals, cleaning and upkeep.

Everyday life

Planning for a return home can include a home assessment at the center or in the patient’s home. The assessment is part of the recovery plan to help prepare for the transition.

Meals are served in a restaurant-style dining room. The center describes a range of dining choices, with chefs preparing food for patients.

Activities & connection

The center has an onsite activities coordinator. Its long-term restorative care includes activities intended to keep residents’ minds active.

Shared spaces & amenities

The therapy gym includes strength-training machines, a dynamic stair trainer and a HUR iBalance machine. The gym is used for recovery therapy.
