Peak Rehab is Salmon Creek’s short-term subacute rehabilitation program for patients, including those with complex medical conditions. It uses advanced equipment and skilled therapists in a seven-day program. Listed service areas include physical, occupational and speech therapy; post-surgical and orthopedic rehabilitation; cardiac, pulmonary, neurological and stroke care; diabetes and respiratory care; wound care, IV therapy and pain management.
Long-term care services include restorative nursing, wound care for slow-healing wounds, dental and podiatry services, and inpatient rehabilitation with supplemental PEG-tube feeding.
Memory care serves residents with Alzheimer’s and other forms of dementia. Specially trained staff use a person-centered approach, with 24-hour supervision, help with personal care, medication arrangement, specialized therapies and incontinence care.
The location also describes skilled-staff services for TPN, advanced pulmonary care and tracheostomy care.
