A medication monitoring system covers scheduled medications, eye drops, insulin, blood thinners and anticoagulants.
Care plans are based on a functional assessment, and the community offers five levels of care.
The community partners with mobile rehabilitation therapies and home health, including physical, occupational and speech therapy, hospice, palliative care and respite care.
