Haven Post Acute

Nurses are available 24 hours a day, and therapists, dietitians and case managers work around each resident's schedule.

The post-acute care program serves residents recently released from the hospital, including those who are medically fragile, with the goal of helping them return home.

Skilled nursing services include IV therapy, wound care, catheter care, enteral feeding, pain management, diabetic management, medication management, and off-site dialysis, coordinated with an onsite medical director.

Dementia care is offered, though the facility is not a locked unit.

A restorative nursing program creates individualized plans in areas such as ambulation, range of motion and activities of daily living.

Physical therapy, occupational therapy and speech therapy are provided; occupational therapy includes a full-functioning kitchen used for training.

The social services team assists with care planning and discharge planning, and can arrange dental, vision, podiatry and audiology services as well as transportation to appointments.

Meals are provided to meet specific dietary and therapeutic needs according to physician orders, with regular nutritional assessments from a registered dietitian.

Each resident receives a calendar of upcoming events such as birthday parties, musical programs and outings, and friends and family are welcome at special events.

Activities include musical performances, fitness and stretching exercises, singing, games, trivia, religious meetings and volunteer programs.
