Clinical services include IV therapy, wound care, diabetic management, pain management, restorative nursing, pulmonary care, bariatric care, stroke recovery, and hospice and palliative care.
Post-acute rehabilitation covers orthopedic rehabilitation, cardiac care, stroke recovery, post-operative subacute care, and physical, occupational and speech therapy, with each care plan designed around the individual.
Orthopedic rehabilitation is a physician-led program with customized recovery plans, therapy seven days a week, and pain and wound care. Complimentary home evaluations, discharge planning and car transfer training support the return home.
Chronic kidney disease care includes an on-site dialysis room, a team led by a board-certified nephrologist, nephrology consultations, laboratory testing and renal diet counseling with a registered dietitian.
Memory care includes on-site physical and occupational therapy, psychology and psychiatry services, sensory and reminiscence therapies, art and music activities, and intergenerational programs.
Respite care provides 24-hour skilled nursing care, medication administration, a life enrichment program, meals and rehabilitation services while caregivers take a break.
Urgent SNF offers short-term stays with nursing oversight and rehabilitation services for patients who do not meet acute admission criteria but still need support.
