Care & support

Short-term rehabilitation helps people restore abilities after stroke, cardiovascular problems, orthopedic surgery and other conditions, with physical, occupational and speech and language therapies and a tailored care plan.

Within the first 72 hours after admission, the Road to Recovery program brings the care team, the patient and family together to identify discharge goals.

Rehabilitation services include wound care, nursing, contracture management, wheelchair positioning, mobility training and walking programs.

The advanced pulmonary program supports patients with conditions such as COPD, tracheotomy, pneumonia and chronic emphysema, with pulmonologist consultation, on-site respiratory therapists and disease management education.

Ventilator care is provided under the direction of a pulmonologist with an individual care plan, ACLS-certified staff, and systems that alert staff when oxygen levels are not optimal.

The memory care program uses a person-centered, wellness-based model with the Meaningful Day activity program and a dementia-supportive living environment.

Long-term care is provided by licensed nurses, specially trained health care aides and an interdisciplinary team, with social services, activities and therapies. Respite stays and hospice care through local hospice partners are also available.
