Care & support
Short-term rehabilitation helps people restore abilities after stroke, cardiovascular problems, orthopedic surgery and other conditions, with the goal of returning home safely after a hospitalization.
Rehabilitation includes physical, occupational and speech/language therapies with support from doctors, nurses, therapists and social workers. Services also include wound care, nursing, mobility training and a strength-building wellness and aerobic conditioning program.
A Road to Recovery meeting brings the care team, the patient and family together within the first 72 hours after admission to identify discharge goals.
Memory care follows a structured, person-centered, wellness-based model with a Meaningful Day activity program built on four domains of wellness and a dementia-supportive living environment.
Long-term care is provided for people who need help with daily tasks and can no longer be cared for at home, by licensed nurses, specially trained health care aides and an interdisciplinary team under the guidance of an attending physician.
Respite stays are available on a short-term basis for people whose home caregivers need a break, and can be extended if more care is needed. Hospice and palliative care is offered in partnership with local hospice companies.
Outpatient therapy is available, with treatments designed to increase mobility and goals set by therapists in coordination with the patient's physician.
